Position Description

Job Title: Registered Nurse – Home Health

Location: Tulsa, OK

Schedule: Full-Time | Days | Monday–Friday | 8:00 AM – 5:00 PM

Pay Range: $33.15 – $46.27 per hour

Sign-On Bonus: $10,000 with 2-year commitment

Travel: Mileage reimbursed at $0.70/mile; reliable transportation required

Job Summary

The Registered Nurse (RN) provides skilled home health care by assessing patient needs, developing individualized care plans, and delivering nursing services in accordance with physician orders and agency standards. This role requires strong clinical judgment, effective communication, and the ability to work independently while coordinating care with an interdisciplinary team.

Key Responsibilities

  • Perform initial and ongoing patient assessments to determine eligibility and care needs for home health services.
  • Develop and revise patient-centered care plans based on goals, preferences, and clinical findings.
  • Administer medications and treatments per physician orders, ensuring adherence to safety and infection control protocols.
  • Educate patients and caregivers on disease management, medication compliance, and discharge planning.
  • Monitor and document patient progress, medication lists, and care coordination in EMR systems using accurate medical terminology.
  • Conduct Home Health Aide supervisory visits every 14 days and update care plans as needed.
  • Collaborate with interdisciplinary teams to ensure continuity of care and timely referrals.
  • Maintain compliance with agency standards, QA processes, and regulatory requirements.
  • Support agency goals through case conferencing, QAPI initiatives, and staff orientation.

Qualifications

Required:

  • Valid RN license (Oklahoma or multi-state).
  • Current BLS and ACLS certifications.
  • Reliable transportation, valid driver’s license, and insurance meeting agency guidelines.
  • Basic computer skills and proficiency in EMR documentation.
  • Strong clinical, interpersonal, and communication skills.

Preferred:

  • Minimum one (1) year of home health or related nursing experience.

Additional Details

  • Flexible scheduling to accommodate patient needs.
  • Travel required to patient homes in Tulsa and surrounding areas.
  • Fingerprinting and background check required.
